Enrollment & Camp Participation
Enrollment is complete once all required forms have been submitted. Lifeline reserves the right to deny or discontinue participation if documentation is incomplete or program policies are not followed.

Attendance & Camp Supervision
Students are expected to attend regularly and follow program guidelines. Lifeline staff provide supervision during scheduled program hours only. Parents/guardians are responsible for timely drop-off and pick-up.

Behavior Expectations
Students are expected to demonstrate respect, kindness, and responsibility toward staff and peers. Lifeline seeks to model Christ-like character and may dismiss a student whose behavior becomes disruptive or unsafe.

Medical & Camp Emergency Care
Parents/guardians must provide accurate medical and emergency contact
information. In the event of an emergency, Lifeline staff may seek medical treatment for your child if a parent/guardian cannot be reached. Any medical expenses are the responsibility of the parent/guardian.

Medication
All medication must be clearly labeled and accompanied by written instructions from the parent/guardian.

Payments & Camp Refunds

Program fees must be paid according to Lifeline’s schedule. Refunds are not guaranteed and will be considered on a case by-case basis.

Photo Camp Media Release
By enrolling, you grant Lifeline permission to use photos or videos of your child for ministry and promotional purposes unless written notice is provided.

Liability Waiver
I understand that my child may participate in any number of activities, some of which include, but are not limited to, recreational activities and games. I understand that there are certain risks associated with any activity. I will assume responsibility for these risks, whether known or unknown to me at this time and whether the risks were caused in whole or in part by any acts, omissions or negligence of Lifeline or its directors, volunteers, employees or agents. I do hereby release Lifeline and its directors, volunteers, employees, and agents from any liability due to accident, physical or mental injury, incurred by my child including death or illness, whether caused in whole or in part by acts, omissions, or negligence of Lifeline, while participating in or traveling to or from any events or activities sponsored by Lifeline. I understand that this means I agree not to sue Lifeline or its directors, volunteers, employees, and agents for any accidents or injuries suffered by my child while attending or participating in a Lifeline activity, whether the accident or injury was caused by any acts, omissions, or negligence of Lifeline. This release is also intended to include all claims of my family, estate, heirs, personal representative or assigns. If I am under 18, my parent or guardian, by clicking to have read above & agree, also consents to my release and he or she agrees that this release shall be binding upon him or her as my parent or guardian as to me and my estate heirs, personal representatives and assigns. My parent/guardian promises, to defend, indemnify and hold Lifeline and its directors, volunteers, employees, and agents harmless from any and all claims or lawsuits against Lifeline, including claims or lawsuits based upon any acts, omissions or negligence of Lifeline or its directors, volunteers, employees and agents, if I should repudiate this release after obtaining adulthood. I understand my child will be transported by school bus on a weekly basis and hold harmless all releases assigned to transport campers in said bus.

Lifeline reserves the right to adjust schedules, activities, or policies as needed to
best serve our families and ministry.
